UOTA Awards

Submission guidelines:

Award recipients will be announced at the UOTA Annual Membership Meeting on Saturday, May 16, 2026, in St. George. Please note that award recipients must be current UOTA members; therefore, we encourage prospective nominees to ensure their membership is up to date prior to nomination.

All nominations must be submitted by March 15, 2026. We appreciate your support in recognizing excellence within our profession and look forward to receiving your nominations.

UOTA Award of Excellence in Practice

This award is to give recognition to an Occupational Therapist or Occupational Therapy Assistant that has demonstrated excellence in practice, scholarship, research, education, and/or collaboration, that has contributed to the enhancement of the profession of Occupational Therapy in the state of Utah.

Please note, all fields are required.

 

UOTA Bonnie Held Award of Service

This award is to provide recognition to an Occupational Therapist or Occupational Therapy Assistant UOTA member who has demonstrated excellence in service to occupational therapy practice, the UOTA organization, and/or the Utah community.

Please note, all fields are required.

 

UOTA OT/OTA Partnership Award

This award is to provide recognition to an occupational therapist and occupational therapy assistant team residing and working in Utah, who exemplify a professional partnership through collaborative efforts that promote the occupational therapy profession.

UOTA Eunice Chen Mentorship Award

This award is to provide recognition to an occupational therapy professional who has demonstrated excellence in mentorship related to the profession of Occupational Therapy in the state of Utah.
